Access this service by logging in at the user account menu (see website header), then scroll down the menu to Confirm My TYPE. Fill out the information form and upload your evaluation pictures (1 side view, 1 front view).
To take the required photos, follow Structural Evaluation Actions 1-5 outlined in the book (Step 6). Upon your photo submission, please allow 2-3 days to receive confirmation of your Bad Back TYPE.
